I was also looking for arena/survival maps like you, But all of them I have found were kinda dull, and I started to realize that Doom doesn't work well with wave-based survival. If you play this mod in random level mode, it offers everything you want : no puzzles, no deep exploration, just you and bunch of enemies cause randomly generated levels are not very complicated.
It features Perma-death, new difficulties, and random level generation. It's not what exactly you are looking for but you'll be satisfied with it. I recommend to try this mod, Manly-ass doom.
